Most of them are available as rewards after long champion level dungeons (intended for heroes that are at least Level 5). The map, bottle, tentacle idol, and candle can only be obtained by defeating The Shambler, a very difficult enemy. He has the same drop rate for rewards on lower levels. Try fighting him in apprentice dungeons if possible. He appears after you place a torch on a curio named "Shambler's Altar". He can also randomly appear as a corridor fight, which is more likely with low torchlight. Successfully complete the indicated task to unlock the corresponding achievement.
